\n Completed visa application signed by the applicant.<\/p>\n

Applications for minors must be signed by both parents. Formal consent from both parents, or from the parent not accompanying the child during the visit, duly legalized by the Embassy of Italy in Colombo, is mandatory. Both parents must attend the interview with the child and present their passports.<\/p>\n

For minors, the translated birth certificate of the minor, legalized by the Sri Lankan Ministry of Foreign Affairs, must be attached to the visa application.\n <\/td>\n

2<\/td>\n\n One recent ICAO-compliant colour photograph<\/strong>

3<\/td>\n\n Current Passport and Previous Passports, if any<\/strong>

4<\/td>\n\n Covering Letter<\/strong>

5<\/td>\n\n Proof of Accommodation or Hospitality<\/strong>
\n Confirmed hotel, apartment or lodging reservation in Italy.<\/p>\n

Confirmed hotel, apartment or lodging reservation in any other Schengen country to be visited, if applicable.<\/p>\n

If the applicant will be hosted, the host must submit a signed and dated invitation letter to the Embassy of Italy in Colombo, together with a copy of his\/her identity card or passport. If the host is a non-EU citizen, a copy of the host\u2019s Residence Permit (\u201cPermesso di Soggiorno\u201d) must also be provided.\n <\/td>\n

\n Confirmation letter from the employer regarding approved paid or unpaid leave.<\/p>\n

Salary slips for the last 3 months and EPF and ETF contribution statements.<\/p>\n

Company business registration, most recent company bank statements for the last 3 months, company income tax payment slips, annual reports, balance sheets and\/or audit reports.<\/p>\n

If the applicant is a student, a letter from the educational institution granting permission for academic leave must be provided.\n <\/td>\n

\n Personal bank statements for the last 12 months, including current, savings and fixed-deposit accounts.<\/p>\n

Copies of vehicle registration certificates, if any, together with their valuations.<\/p>\n

Any bank financial guarantee issued in favour of the applicant.\n <\/td>\n

8<\/td>\n\n Confirmed Hotel Reservation<\/strong>

  • The Embassy reserves the right to request further documentation if and when required.<\/li>\n
  • All documents should be provided in original, accompanied by one set of photocopies in A4 size.<\/li>\n
  • Documents in Sinhalese, Tamil or any other language must be presented with an official English translation and certified by the Sri Lankan Ministry of Foreign Affairs.<\/li>\n
