Registration of a Birth Certificate

To request the registration of a birth certificate in Italy of a birth certificate, the following documents must be submitted:

1. Application for the transcription of the birth certificate, using the updated form;

2. Birth certificate or certified extract copy issued by the competent Civil Status Office of Sri Lanka or the Maldives. The document must be submitted to the Embassy already legalized by the Ministry of Foreign Affairs of Sri Lanka or the Maldives. 3. Photocopy of the applicant’s passport or valid Italian identity document;

4. Documentation proving possession of Italian citizenship.
