VISA CHECKLIST

FAMILY REUNIFICATION yes no n/a
1) Application Form

  • Completed visa application signed by the applicant;
  • Applications of minors must be signed by at least one of the parents or legal guardians.
2) Two recent passport size colored photographs

  • Full face on white background, one photo attached to application.
3) Current Passport of the applicant and family member in Italy

  • Passport must have at least two blank pages;
  • Passport must be valid for a period of at least 3 months beyond date of submission;
  • Copy of current and previous Passport bio-data page.
4) Family Reunification Authorization (“Nulla Osta Ricongiungimento Familiare”)

  • Original certificate (plus a copy) through which the competent “Prefettura” (Sportello Unico per l’Immigrazione – S.U.I.) authorizes the applicant to reunite with the family member residing in Italy. Please note that the “Nulla Osta” is valid only 6 months from the date of issuance.
5) Documents of the Family Member residing in Italy

  • Copy of Passport bio-data page (pages 2,3,4,5);
  • Copy of valid Permit of Stay (“Permesso di Soggiorno”);
  • Copy of Italian Identity Card.
6A) SPOUSE CATEGORY ONLY:

  1. Original marriage certificate (plus a copy) duly legalized by the Embassy of Italy in Colombo. In cases of surname changes after marriage, it is mandatory to indicate the “name before marriage” on the passport. This should be done without any corrections or alterations. For those who got married in Italy, the original Italian marriage certificate issued by the competent Italian Municipality (Comune), plus a copy.
  2. Photos of marriage ceremony/marriage registration;
  3. Proof of remittances (Italian receipts).
6B) SONS/DAUGHTERS CATEGORY ONLY:

  1. Original birth certificate of the applicant (plus a copy) duly legalized by the Embassy of Italy in Colombo. For individuals born in Italy, the original Italian Birth Certificate that includes the full names of parents plus one copy.
  2. For minors below the age of 18, the application must include the original formal parental consent to expatriate from the other parent which must be signed at the Italian Embassy in front of an Embassy officer. In the event that one or both parents are in Italy, the signature (of those who are in Italy) may be authenticated by an Italian public officer delegated to this function (e.g. municipal officer) or, alternatively, by an Italian (Notaio) Notary Public.
  3. If both parents are residing abroad, in addition to the formal letter of consent to expatriate, they must formally authorize a guardian to submit documents for the visa application, notifications, and collection. The letter appointing the guardian must also be signed in front of an Italian public officer delegated to this function (e.g. municipal officer) or, alternatively, by an Italian (Notaio) Notary Public.
  4. Copies of documents belonging to the other parent, including their residence permit (if residing abroad), as well as their address and contact telephone number.
  5. Copy of documents of the other parent, residence permit (if abroad), address and contact telephone number, need to be attached to the application form. Similarly, copies of documents pertaining to the guardian or guardians must also be included, along with their respective address and contact telephone number(s).
6C) DEPENDENT PARENT CATEGORY ONLY
(The applicant must demonstrate that he/she has no other children in Sri Lanka or, if he/she is over 65 years of age, in the event of the presence of other children in Sri Lanka, he/she will have to demonstrate that they cannot take care of his/her support due to documented, serious health reasons)

  1. Birth certificate of son/daughter residing in Italy, original (plus one copy) duly legalized by the Embassy of Italy in Colombo;
  2. “Certificate on Residence and Character” issued by the Grama Niladhari (DS04 form) and legalized by the Embassy of Italy in Colombo. If the applicant has no other children in Sri Lanka, the Grama Niladhari should declare that he/she has personal knowledge that the applicant has no other children in Sri Lanka and should not simply state what the applicant has declared.
  3. Proof of financial support from son/daughter residing in Italy. Original receipts of regular bank transfers of the past years must be submitted. The economic remittances made by the son/daughter for the benefit of the applicant, in terms of quantity, frequency and amounts transferred, should be sufficient to demonstrate economic support of a structural nature aimed at meeting essential needs and should demonstrate the complete financial dependence of the applicant on the son/daughter residing in Italy;
  4. Passport/ID copy of the other sons/daughters in Sri Lanka. In the event that the applicant has other children residing outside Sri Lanka, the applicant must provide copies of foreign residence permits and identity cards and/or driving license that demonstrate that these children legally reside outside Sri Lanka;
Please note:

  • The Embassy reserves the right to request further documentation if and when required;
  • All documents should be provided in original accompanied with a set of photocopies in A4 size;
  • Documents in Sinhalese, Tamil or any other language must be presented with an official English or Italian translation and must be legalized by the Embassy of Italy in Colombo. In case of non-Sri Lankan nationals, documents must be certified by the respective Embassy / High Commission / Consulate based in Colombo.
