1 Application Form
Completed visa application signed by the applicant. Applications of minors must be signed by at least one of the parents or legal guardians.
2 One recent ICAO-compliant colour photograph
Full face on white background. The photograph must be attached to the application form.
3 Current Passport of the applicant and family member in Italy
The passport must have at least two blank pages and be valid for a period of at least 15 months. Copies of the bio-data pages of the current and previous passports must also be provided.
4 Family Reunion Authorization (“Nulla Osta Ricongiungimento Familiare”)
Please note that the “Nulla Osta” is valid for only 6 months from the date of issuance.
5 Documents of the Family Member residing in Italy
Copy of the passport bio-data pages (pages 2, 3, 4 and 5), valid Permit of Stay (“Permesso di Soggiorno”) and Italian Identity Card.
6A Spouse Category Only
Original marriage certificate, plus one copy, duly legalized by the Embassy of Italy in Colombo. In cases of surname changes after marriage, it is mandatory to indicate the “name before marriage” on the passport, without corrections or alterations.

For those who married in Italy, the original Italian marriage certificate issued by the competent Italian Municipality (“Comune”), plus one copy, must be provided.

Photos of the marriage ceremony or marriage registration must also be submitted.

6B Sons/Daughters Category Only
Original birth certificate of the applicant, plus one copy, duly legalized by the Embassy of Italy in Colombo. For individuals born in Italy, the original Italian birth certificate including the full names of both parents, plus one copy, must be provided.

For minors below the age of 18, the application must include the original formal parental consent to expatriate from the other parent. It must be signed at the Italian Embassy in the presence of an Embassy officer.

If one or both parents are in Italy, their signatures may be authenticated by an Italian public officer delegated to this function, such as a municipal officer, or by an Italian Notary Public (“Notaio”).

If both parents reside abroad, they must also formally authorize a guardian to submit the visa application documents and receive notifications and the applicant’s passport. The letter appointing the guardian must be signed before an Italian public officer or an Italian Notary Public.

Copies of the other parent’s documents, including the residence permit if residing abroad, address and contact telephone number, must be attached to the application form. Copies of the guardian’s documents, address and contact telephone number must also be included where applicable.

6C Dependent Parent Category Only
The applicant must demonstrate that he/she has no other children in Sri Lanka. If the applicant is over 65 years of age and has other children in Sri Lanka, he/she must demonstrate that they cannot provide support due to documented, serious health reasons.

The following documents must be provided:

1. Original birth certificate of the son or daughter residing in Italy, plus one copy, duly legalized by the Embassy of Italy in Colombo.

2. “Certificate on Residence and Character issued by the Grama Niladhari” (DS04 form), legalized by the Embassy of Italy in Colombo. If the applicant has no other children in Sri Lanka, the Grama Niladhari must declare that he/she has personal knowledge of this circumstance and must not simply report the applicant’s declaration.

3. Proof of financial support from the son or daughter residing in Italy. Original receipts of regular bank transfers made during the previous years must be submitted. The quantity, frequency and value of the transfers must demonstrate structural economic support aimed at meeting the applicant’s essential needs and his/her complete financial dependence on the son or daughter residing in Italy.

4. Copies of the passports or identity documents of the applicant’s other sons or daughters in Sri Lanka. If the applicant has other children residing outside Sri Lanka, copies of their foreign residence permits and identity cards and/or driving licences must be provided to demonstrate that they legally reside outside Sri Lanka.

Please note

  • The Embassy reserves the right to request further documentation if and when required.
  • All documents should be provided in original, accompanied by one set of photocopies in A4 size.
  • Documents in Sinhalese, Tamil or any other language must be presented with an official English or Italian translation and legalized by the Embassy of Italy in Colombo.
  • For non-Sri Lankan nationals, documents must be certified by the respective Embassy, High Commission or Consulate based in Colombo.
